To make a request:
- Ensure you know which vet and area to request an appointment from (map at bottom of this page).
- Complete STAGE 1 form (on this page).
- Complete STAGE 2 form (emailed out to you immediately after submitting this form).
PLEASE NOTE – both forms need to be completed for a request to be processed – if you do not recieve an email within a few minutes there may be a problem with the email you entered or it may be in your SPAM folder.
Stages of AHC request:
1. STAGE 1 Form – complete the request form above (this is to allow us to arrange an appointment with your desired vet and AHC provider).
2. STAGE 2 Form – complete the form emailed out to you (this gathers the details required for the AHC itself and ensure you are recieving our emails).
3. Your vet emails the rabies vaccine history (you contact them to ask them to do so)
4. You will recieve an appointment offer.
5. You confirm appointment offer is suitable.
6. Invoice issued to you after our checks.
7. You pay invoice to secure the appointment.
8. You meet the vet (with your pets) for the final checks and leave with the AHC.
